Dear Sir/ Madam,

I Mr. Sudheer Kumar working in Axis Bank as a Assistant Manager in Bhimavaram Town Since September 2015 with Employee Number 102934. I was in return journey from my home town Gollagudem on 20/11/2017. During My journey i Lost my Travelling Bag which is hanged to my motor cycle. I have kept my employee ID card in the bag and the same was lost. For Applying a new replacement ID card as per bank policy complaint should be lodged.
