Dear Sir,

I Sauvik Kumar Saha, A/C Holder of above account has sbmitted old Debit Card to Allahabad Bank, Branch - Sodepur, Dist - 24 Parganas (N), Kolkata - 700110, West Bengal in the month of Sep 14 due to mutilation with a request for another Debit Card. I have personally visited for follow ups for issuance or status for my request.

Very unfortunate that all most 3 months gone and I have not given with the new Debit Card and even don't know whether it has been processed or not.

Request your honour to intervene and issue me the Debit Card at the earliest.
