Dear Sir,

On 17.05.2018 at around 8.30 p.m. I have lost by BSNL Sim Card in transit while inserting the same to my new mobile purchased from M/s. Pai International, near Mathikere. The lost Sim had the Mobile No.********84. Hence, I am lodging this complaint. Now, I intend to purchase a new Sim Card in place of the lost Sim Card by blocking the same from BSNL by continuing the same number.

Kindly acknowledge the complaint and do the needful.
