This is to bring to your kind notice that I've been employee in your establishment with EC No. 20028824 and resigned on 14th Oct,2011.

Subsequently I've applied for my PF withdrawal against the PF A/c No. MH/46459/20028824 and simultaneously for Pension Scheme Certificate against my EPS No. MH/45459/25806 held with RPFC, Bandra.

However, to my utter surprise instead of providing my Pension Scheme Certificate , the fund accumulated against it was credited in my State Bank S/B A/c which I had not opted for.

I, therefore request to arrange for issuance of my Pension Scheme Certificate and oblige.
