Sub: Lost of Mobile Phone no.********42 - duplicate SIM requested – Reg.


I have lost my Mobile phone No.********42 on 14.07.2010 while riding the vehicle at Perambur, Chennai-600 011. I request you that the above Sim card may be de-activated immediately and arrange a duplicate SIM Card for the same number may please be given for my regular use.
