I Bhagat ram s/o Kundan Lal r/o - flat no. - 8 famna aptt. sector - 9 Rohini Delhi - 110085 have lost my shre certificate
of axis bank folio no. UTI - 023801 . I have already intimate already 2 times before sending of this mail within 3 months andi have already intimate from phone calis approx 50 times . i have two problems


1. loss of share certificate
2. loss of dividend year 2010- 2011


But from reply they have mentioned only about loss of dividend year 2010- 2011 .
so please i humble request to you please send me the necessary documents required to reissue the share
certificate. u can mail the papers required tomy mail id
