Sub: Error in scheme certificate No.MH/BAN/66824
Ref: My letter dated09.02.11 sent to you by Speed Post No.EN********8IN dated 10.02.11

This reference to my letter cited above (Copy enclosed for your ready reference) on the subject. From the scheme certificate, You will please observe that

“past service up to 15.11.1995 is mentioned as 8 years Instead of 17 years”

In support of my above I submit the following:

01. Letter dated 28.06.1989 of RPFC, Bhubaneswar transferring funds(PF Accumulations
from July 1979 to April 1982) from Bhubaneswar to your good office as proof that I was a member since July, 1979.
02. Form No.10-c where in I had mentioned Date of joining as 04.07.1979
03. Appointment letter dated 18.08.1982

All the above confirm that I was a PF member since July 1979. But in Scheme certificate past service upto 16.1.95
is mentioned as 8 years instead of 17 (There was a lock out from 30.1.2016 to 08.01.89. Probably this date might have
Considered which is wrong)

In view of the above, I request your good selves to issue amended scheme certificateAt the earliest as I am nearing to retirement. On hearing from you, I shall send the Original Scheme certificate with me.

Inspite of several reminders dated 17.11.13, 16.12.13 and 21.02.14, issue is yet to be solved.

I AM RETIRING ON 15.06.2014, BUT UNABLE TO SUBMIT PENSION PAPERS WITHOUT SCHEME CERTIFICATE.
