Sub: loss of share certificate

Dear Sir/Madam,
I Mr. Sivaprasad S/o Late Gangappa Sivanna My father holding Ms. MYSORE VEGETABLE OIL PRODUCTS LTD shares Folio No. 50000001 total 584 shares and now the Gangappa Sivanna has Expired on the date of 21/04/2002 we have surched the documents after he death we could not find the douments. Now we request to be issue duplicate shares certificate for climing purpus.
