My Mother Mrs. S V Padmaja is a central Government employee working in Accountant General's office, Bengaluru. The CGHS(Central Government Health Scheme) card allotted to my Grandmother (Mrs Gowramma age 87, mother-in-law to my mother) has been lost. Card was missed in and around the premises of the Accountant General's office, Park House Road, Bengaluru, a month back (5-6th October approximately).
