Regional Passport Office - Passport not issued since Jan 2011

Hello Sir / Madam
I have applied for my dad's passport in Jan 2011 , till now we havent recieved the passport. Below are the details
File No : BNGG00029211
DOB : 04/05/1951
As of now it shows the status as "Passport Under Review at regional passport office".
My dad personally went to the koramangala office to file a complaint and he was told passport will be sent in 10 days, which hasn't happened yet.
For a short while the status of the passport was showing as "Passport Granted" and then it changed to "Passport Under Review at regional passport office". Not sure whats happening.
Pls help as its urgent now.
