As i was having a mobile connection in my name D Dhamodaran (post paid BSNL Sim) and the number is ********78 which was used by me with XOLO one HD model handset and the phone is been lost (with SIM) while travelling in BMTC Bus from Shivajinagar to Maruthisevanagar on 06/01/2017 at around 2:00 pm.
since i request to issue the complaint acknowledgement to enable me to procure a Duplicate SIM for the lost number from BSNL.

Phone details:
It is Black in color. It had a white back cover made of silicone.
