ICICI - ICICI bank records shows new ATM card issued.I Hav'nt recvd since 29 August2012

Sir/Madam

I had requested for Issue of new ATM card against damaged ATM card on
29 August 2012 for Saving Account No. ********545 at my Branch Sector
8 Rohini,Delhi 110085 .

Being a senior citizen it's very difficult for me to visit branch at
regular intervals.

Today(2nd November 2012) I revisited the branch (Sector 8 Rohini) to
know the status of the request for New ATM card.

Today I am stunned to know from your employee at Privileged Bank desk
staff that the card has already issued and delivered to my address.

Kindly inquire as to where the card has gone because I have not
received any ATM card after the request filed on 29 August 2012.

Now the employee told me that the ATM card which is not recd by me is
BLOCKED by her .I need an assurance that no such FRAUD has been done
by your side against that ATM card which is never Issued or never
delivered to my address.

Now a new request No. is generated ( SR 244716531 and SR 244705571).I
am not able to understand what's going on in ICICI Bank ATM card
Issuing Division.

Please Note: Attachment showing Acknowledgement receipt issued by
ICICI sector 8 Rohini Delhi Branch on 29 August 2012 for Issue of New
ATM card .
